You can also pay to have your clothes washed. Oh and going to a bath house gets you fully clean. I keep my gear repaired but even now I need to hit up a armor smith and have the dents fixed as well as cleaned up. Its hard to see in the character screen but I have quite a lot of blood on me. Also blood will get on your clothing and armor and the more you fight the worse it gets. And when you fight your clothing and armor take damage and show it. You can actually start to stink from BO and such.
You can wash up your face by using a water trough but it doesn't do much for you.
One thing that is cool is how your appearance makes a difference in how people treat you.
